Our Aftercare Management Services
The focus of our services is on in-patient and out-patient treatment cases in psychiatric and psychosomatic medicine. By telephone, we get in touch with the physicians and therapists and with the hospital social services, so that the care demand can be discussed at an early stage and appropriate action can be initiated.
Our intervention is usually timed during the first days of treatment and is mostly done through telephone contacts with the attending hospital doctors or the hospital social services.
During in-patient treatment, our services also include communicating with other potential cost carriers, such as pension providers and others – should long-term care be needed, or if there is a need for follow-up measures such as rehabilitation admission or a transition to supervised living arrangements. Upon request, we can also conduct inspections at the health insurance’s medical services.
